Individual Service Coordinator (ISC) / Case Manager

Location: Chicago, IL


Job Description

To qualify you must have the following requirements:

The Individual Service Coordinator (ISC) /Case Manager reports directly to the Senior Program Manager or Program Manager regarding the on-going operations of her/his caseload. This includes the provision of case coordination service to eligible recipients and the administrative processing of referrals from the community. Additionally, the Case Manager is responsible for interfacing with staff of numerous community agencies providing day, residential, support, generic services to clients, as well as writing the Person Centered Plan.

Must have Bachelors degree in a human services field including but not limited to sociology, social work, special education, rehabilitation counseling or psychology.

Must have QIDP Certification and verification of 40 hour QIDP training completion.


Preferred:

Bilingual English/Spanish or Bilingual English/Korean

HOME BASED ISC – Requires at least 1 year of experience working directly with individuals with an intellectual disability, or a related condition such as autism, or cerebral palsy. Must also have experience in any of the following:


In addition, all candidates must be computer literate and possess good English, phone, interpersonal, and communication skills, must also have own reliable transportation. Team players only please!
