MATERNAL AND CHILD HEALTH SERVICES
CAU is a licensed Maternal and Child Health organization by the State of Illinois. CAU provides the Family Case Management (FCM) program to assist pregnant women and infants to access quality medical care in an effort to reduce infant mortality rates in Illinois. This program targets pregnant women who are economically challenged and eligible to receive Medicaid. Each year our qualified case managers serve over 3000 women and infants who may reside in the city of Chicago and the North and West Cook County. CAU provides a multi-lingual team who deliver culturally competent case management services to meet the diverse needs of families. CAU case managers work with new mothers to connect families to appropriate medical care, eligible public benefits, support services, and community resources. They also provide information and referral regarding prenatal care, checkups and immunizations, breastfeeding, infant development, family planning, parenting, and other common baby matters. The overall goal of FCM is to help each family fulfill their goals for their children by helping them achieve a healthy start for their new baby.
CAU provides the Intensive Prenatal Case Management (IPCM) Program that may also be known as the Better Birth Outcomes by the State of Illinois. The focus of this service is to provide case management for pregnant women with high risk pregnancies who are economically challenged and eligible to receive Medicaid. The CAU case manager provides enhanced services to women at increased risk of premature delivery or any other potentially adverse birth outcome. The CAU case managers are required to be either a registered nurse or a licensed social worker. During frequent home visits and attentive follow-up, our staff provides state-of-the-art prenatal health education developed by the March of Dimes. CAU case managers closely follow up with the expectant mother in a personalized approach, supporting her efforts at positive change and her preparations for having a healthy baby.
