Community Alternatives Unlimited (CAU) provides service coordination or case management for persons enrolled in the Illinois Division of Developmental Disabilities (DDD) Home and Community Based Waiver Services. As an Independent Service Coordination agency (ISC), CAU collaborates with individuals and service providers to ensure health, safety, welfare, well-being, and satisfaction with services funded by Division of Developmental Disabilities.

Here’s More You Should Know About the ISSA Program:

  • The person(s) enrolled in the program may live with family, semi-independently, or in a 24 hour waiver funded group setting.
  • Every person in the program is connected to one of our ISSA case managers, and receives face to face visits per waiver regulations. Face to face visits are a requirement of the program.
  • Annually, person(s) receiving services will complete a Person-Centered Plan. This plan reflects an individual’s preferences and support needs. As a part of this process, assessments are conducted of individual service needs, which may include referrals to Vocational Rehab for individuals interested in competitive employment in the community.
  • ISSA case managers monitor the implementation strategy created by the provider agency. They ensure that the participant’s general health, safety, and well-being is met, and that the individual’s chosen outcomes are achieved.
  • ISSA services are designed to assist but not supplant the guardian or other appropriate advocates in fulfilling responsibilities.
  • ISSA case managers serve as an independent resource for consumers and families to clarify issues with providers or services, address emergencies, facilitate changes in support levels and provide conflict resolution.
  • ISSA program services are provided at no cost to those enrolled in waiver services.
